The form may be returned in-person at the Jr./Sr. High School office or completed and emailed back to cheil@uvstorm.org.
Also, please remember the Fall Sports Meeting on Wednesday, Aug. 13, from 6 to 7 p.m. in the Auditorium. This meeting is for both parents and student-athletes.
